Aztec Death Whistle

The Aztec Death Whistle is a skull-shaped whistle used to vanquish Death Reaper by emitting a screeching sound.

To use it, simple raise the skull to your mouth.

To vanquish the Death Reaper you must be in the same room and within about 20 feet of it.
